Insurance Companies Out Of Network

If you have insurance coverage in an Out of Network Company we will be happy to submit your insurance claims to your company. We will await payment from the Out of Network company but projected co-pays will be due at the time of service. This is a projection and our best estimate. It is possible to receive a statement afterwards. We try our best to be as accurate as possible but with being out of network we do have limitations on information.

We do not participate with Medicaid or DHMOs
